Bug? Rise of the Black moon 2 doesn't spawn?

So I'm currently trying to get all the golden collectibles for the true ending

For some reason rise of the black moon 2 doesn't seem to spawn for me.

I triple checked. I am at the right mission, and at the right place and it's not in my inventory, but for some reason it just...isn't there...

Same issue for me, on my second playthrough and wanting to get the true ending I've been running around, this is the one thing I can't find. User guides suggest it'll be in a certain location, but it's not there in my playthrough.

They swapped locations, for exemple black moon rose 3 is at 1, prophecy 4 is at 2, check all locations
