Counter is for the special attacks and you should absolutely use it when such attacks pops out.
Phase 2 is the easy part here, just settle down and play it safe. Viper is no longer supper aggressive other than his smoke/dark phase. The boss is doing a lot of 1-2 attacks which you can brink block and get a hit or two in. Do not get greedy, just whittle his health down. You will have to go through at least 1 smoke/dark phase--this is all about getting into the phase with enough HP and stamina so you can block/brink block (ideally) his lunge attacks.
As the OP mentioned, the javelins are very useful here. You should have plenty of spirit so use it. The boss is often at a distance where it is safe to chuck one or two javelins. I was hitting head-shots for around 3k damage with un-upgraded javelin skill.

Took me around 30 attempts, my preference was the spear, but yeah felt good to finally beat him. If you can get to phase two with at least 2 heals then I think you're all good as he is slower. There are also the willpower orbs to fall back on for a good heal but they are in short supply and you have to have three spirit to use one.
