55/56 puzzle pieces, all three areas say "complete"

I seem to be missing ONE puzzle piece even though the map says all three areas are "complete." I know you also get puzzle pieces from side quests, which I think I have finished (all listed quests are checked off).
I haven't faced the last boss yet, does he give a puzzle piece? Because I don't know what else I can do.
EDIT: beating the last boss (who isn't the last boss) unlocks one more quest and one more puzzle piece.

I was have the same issue, checked the maps 3 times lol, room per room, and I enjoyed, for me the last one was above on a elevator, almost hidden by the UI, try there you didn't yet!
Sorry I don't remember the map now :(

It's in the hospital area, if you warp into that room it'll place you on top of the elevator and you can grab it.
