Legand of the west outfit bug?

Is anyone else experiencing a potential bug with obtaining the legend of the west outfit? I completed all ambient challenges and got to rank 10 in all of them and the outfit did not unlock for me. I am trying to 100% the game and i am worried that this will prevent me for getting 100% I am not sure what to do. I have doubled checked if i had everything i need and i do. I also saved the game file and reloaded it hoping it would unlock the outfit but no luck.

There's a glitch in undead nightmare as well saying you have 12 out of 13 outfits but the problem is there is only 12 outfits and as a result you're permanently stuck on 98% completion preventing you from getting the zeds dead achievement I've had it happen on 2 playthroughs so far.
