The fire elemental monster (Big fire elemental) has an aura which hits for 5 damage per turn so all your copies died instantly.
This monster is hasted and can hit you for about 20 damage per hit so I assuming he attacked twice then that would be about 40+5 from the aura.
The standard fire amulet has 5 resistance, so it should be about 30 damage after resistances.
Unfortunately it's hard to know without your exact values but he could have potentially hit for even more damage if both of his attacks were critical hits.
Ring/potion of regeneration? Cursed scroll? Fear or confuse effect?
Maybe two "hasted" criticals... or that cursed scroll? I'd swear even the Monster wasn't in melee with me, but anything was possible.

Is it possibly you got feared or some other effect, and so it was more than one turn that passed? I didn't know this until recently, but for the monster elementals you will need to roll a fear check to attack them if you're not fear immune. That along with what everyone else mentioned could be what happened, especially if you were vulnerable to fire due to the Trollish bloodline or a regeneration ring/potion.
I realized that what had happened was that it charmed me, which caused a loss of control to my character. I also have fast animations enabled, so basically when I had been controlled, the game more or less skipped my turns and made the death seem instantaneous.
In actuality it had shot me about 3 times.
In your case, i would speculate you were (as phyre pointed out) feared or maybe confused from a critical strike. Either one causes that loss of control and will skip your turns.
Its a sucky way to die, but after a few deaths you learn which immunities are most important and by what point should have them. Good luck!
