EDIT: Yep, that totally fixed it. WAY more accurate now that I know exactly where the shots are going.
I've definitely had a couple cases where I got punched & kicked to death when my enemy wasn't even close to me, as well as taking cover behind a wall only to get shot anyway.
My guess is that the hit detection is client-sided, which is fine is everyone's got low enough ping. Problem is, someone with high ping will just see someone try to run up behind a wall and shoot him dead, when in fact he was behind that wall. I don't think you can actually fix this one except making sure they got low ping.
For now though, lets just keep the community investigating. I'm sure someone will spot something odd and put up screenshots/videos of it.
Melee sniping as I like to call it is my major gripe with all Source games especially FoF and TF2, it's really terrible being killed 20ft away by someone when you would have totally had them otherwise which only supports my theory of unsynced hit boxes or just glitched hit boxes in general but this is a Source Engine issue although it seems worst in FoF.
