Ryley microwaved a nutrient block for too long and it caused the Aurora reactor to overload.

I mean come on... It's Alterra!
Why should they waste time and money on training people when they can simply write shoddy manuals of how to operate a spacecraft?

Lack of land mass caused optical illusion that they were further from the planet than they thought. Of course it didn't help that Neebs & Co. at Alterra HQ were on an extended lunch break when Aurora requested planetary info.
