Because the game logic for pirates is a bit complex, they can get stuck on different loops. You can sometimes force them out by giving them different orders. Naturall,y as above you need to make sure every top-facing piece of your ship is acessible, because poop in unreachable places can give you a lot of trouble aside from being a permanent hit to crew morale.
If it's any consolation, as long as your cleaning crew is in the ship crew group and not incapacitated, low on morale or hungry they will usually resume their duties eventually.
