What are memory rooms?

When you see a person writing on a notepad(a ghost), follow him after you finish a room, he will keep leading you to the memory room(he will walk into a wall) where you will find coins/memory pieces/ or even fight him for a item.

Are memory rooms generated only when there is a ghost on the level or do they exist without needing a ghost?

They only exist if there is an Archivist on the floor. Hence why there is a level up book that doubles the chance of a memory room spawn on each floor.

Walk through the wall after him, that is where the memory room is.
