You can't tell for sure when someone's used it, but you can make educated guesses that are more likely to be right than wrong. Though you might still be wrong.

If you simply memorize where everything is and how to solve everything, after some trial-and-error, it's really not that hard. I can consistently get through the first few levels without too many deaths.

The "difficulty" comes solely from approaching the game as a normal "react to things as you see them" game, as opposed to one where you need to remember what sorts of unexpected slapstick comedy the game throws at you.
