It is possible to complete the game in about 30 minutes without using any external programs. Some speedrunners can do it, but it involves a lot of tricks, skipping huge parts of the game and at least one of the tricks requires death.
So as far as I know 30 minute finish with 0 deaths or 30 minute finish with 100% shouldn't be possible. Also once there was some guy in the scoreboards with 100% and 0:00:00 time.

The speedrunning community has a branch called "100%", in which they must finish the game 100% as quickly as possible. Rules on what they can or can't do to get that 100% are usually non-existant. Therefore, a game that's speedrun is often explored for glitches or bugs that may break the game completely to get a faster time.

The current World Record for 100% is 1h12, so I'd say everyone faster than that time is guilty of tool-assistance until proven innocent.

More about Ori speedrun times can be found here: http://www.speedrun.com/ori
Record for Any% speedrun is 24m03s at the moment of writing this. There seems to be 4 categories for speedruns on that page for Ori (Any%, 100%, All Cells, All Skills No OOB) and each have different kind of rules.

when it says 100% it means that you have uncovered the entire map it doesn't mean that you have gotten every thing.

Everything goes in the Any% branch. Breaking the game is included, and in most cases even recommended. I believe the most famous example has been A Link to the past, which could be speedrun in any% within 3 minutes.
