Have you checked the "Disable Birth and Death" option on the Difficulty settings when you created your campaign? If yes, this prevents the character from having children. If not, this may be due to your character's and spouse's ages. Waiting in the same settlement as your spouse is enough for pregnancy, but the waiting time might increase with age. If you don't have any mods and did not check the "Disable Birth and Death" option, you can upload your save file to upload.taleworlds.com so we can check if there are any problems.
