You could also implement a Relations influence based on a new Trait (Open-Minded / Hidebound), which would improve or erode relations with other clans based on that Clan Leader's outlook, and that could extend to Cultural differences as well.
So if Derthert had the Hidebound trait, and was himself Homosexual, and your newly minted character were a Hetero Battanian with Open Minded, you would start off with a fairly solid negative relations with deyCortain (I think that's D's clan, top of my head), but if you were a Homosexual Hidebound Vlandian, you would start off in good shape with his clan.
But like I said, the platform is nowhere near being ready for this, and a sizable chunk of the player base is definitely not mature enough for it.

But, if they have forbidden certain subject matter from games or there is a very strong.. inducement for TW to avoid including certain sorts of gameplay, that is not beyond the realm of sensibility.
And, to be very fair - No studio must include mechanics in their game that they don't want to include. So very many studios catch a heck of a lot of flak from all sides no matter what decisions they make regarding any accepted or unaccepted social or cultural practice...
If one avoids the fire, one does not get burnt.
Still, the game is a battle simulator not a dynasty simulation. The latter would be crusader kings. The relationships in the game do not really matter. By the time your family is strong enough to matter in the game, you most likely already conquered the entire map or are so powerful, it no longer matters.
