I grab the brother in the army of the main hero - the only reward in the whole main story - and forget it completely! There is no disadvantage for letting expire the main quest.
The main story is boring, imbalanced and it was concepted for totaly different game!
The cool kids play the campaign game so they can rescue their siblings, which are three of the best companions in the game and which are not available in sandbox mode. But then, they ignore Neretzes' Folly and let it expire unfinished. This kills the main quest, but no harm is done.
If you ignore the main quest, you can just play it like a sandbox.
That main quest is not necessary to establish your own kingdom, and that banner doesn;t do ♥♥♥♥ for you. (You can easily find better banners.)
You, too, can be a cool kid.
Or you can rise to the challenge. Not like the game offers much of those really. With how the game now handle factions when they lose their last fiefs, it's a nothing burger..
