clans leaving faction
all clans i recruit leave the same day i recruited them. What's up with that?

Unfinished game features i'd say. I'm sure a fix will be patched in soon.
I havent seen this happen yet but people keep bringing it up. Can you give more details on your savegame? Which patch was it started? Which clans (minor merc?)? Are you modded and how much rep do you have with the clan? Was it built rep or dev consoled?

Fiefless clans will leave a faction. Quite annoying really as they will leave almost straight away, meaning the faction will become weaker. Luckily you can get them to rejoin you again without having to persuade again, once you get some fiefs to give to them.

The problem is on my end i can't even grant them one single fief.

Fief'd clans will leave your faction too (and take their fiefs with them)

Download a mod or only use mercenaries

You need a mod to grant fiefs. You also need a mod to fix the defection. Download Diplomacy Fixes and Defection Overhaul. The game is nigh unplayable without them. If youre a king its not nigh unplayable, its straight unplayable without them.

So basically you need mods to fix the stuff that the devs cant or wont. Some peoples modlists on the subreddit are literally DOZENS. Not sure how or why TW thought to release such a broken, buggy mess.
You dont need mods. Ive got 2 campaigns and both have clans stay for me hundreds of days and never leave. I expect people aren't building relationships appropriately and/or having mod conflicts.

For one thing, make sure you recruit lords with similar behavioural traits (honour etc) and high rep (+30 at least). Also make sure they aren't minor factions (if you read their desciptions you'll notice the monir clans are rogues and bandits).

I've read from modders that relationships isn't actually programmed to do anything
1 and 100 is the same result
I've heard the same but aim for it anyways until someone figures out exactly why people keep having this issue. I never have. Minor lords leave when contracts expire, but thats it. I just renew.

I found out there is some relation about your kingdom size and number of clans. If your kindom (number of fiefs) is too low comparing to number of clans some will leave. And you even can not hold them by giving a fief. The last time i had for around 300 days no clan leaving me. But soon after i lost a city and a castle one clan did leave.

Anyway, clans should leave only under extreme circumstances. Just because they dont have a fief for a month is not such a circumstance.

When you join a kingdom as a vassal you have to swear your loyalty to the king. Has anyone ever read that oath you give when you join a kingdom? To break that oath means treason against the crown. The sentence is death.

To reflect that, desertion should be very rare. You shouldnt be able to buy or charm, without extreme difficulty, your way into clans deserting for you, or vice versa.

But when it does happen, that deserting clan should be free executions without relations penalties (of course war could happen) if caught by the original faction.

Of course, one of those extreme circumstances is if a kingdom is taking a beating, and the rats are leaving the sinking ship believing that the ship will actually sink. But if it doesnt, then payback should be on the menu.
