Yeah the choice changes nothing directly but i think if it were to be a main choice then it affects the next episode with your relationship with kenny, so if you try to save him i guess he'll say "thanks for what you did back there" or something but if not then... he still might say thanks for protecting sarita
The only way to get hit by Troy in the face is when you try to help Kenny.
If you help Carlos restrain Sarita then you succeed and no one gets hit (except for Kenny of course).
If you stay still and do nothing then Sarita overpowers Carlos (!) and goes to help Kenny herself (go Sarita!) but is hit by Troy and she gets the cheek injury instead of Clem.
