If you install the Unofficial Patch, did you start a new game with the patch, or did you try starting with vanilla game icon itself ?
If installing the patch, you must / need to start a new game from patch not the vanilla game ! =)

Switch off "environmental effects" (or whatever the option is named).
Switch off "high quality audio"
Disable Steam Overlay
setting "core affinity"
Re-install
There was a thread in which Wesp5 and Vepar, solved a bug by opening the game console while the game was stuttering, and examining the repeating error messages that appeared in the console window.
I tried un-checking the environmental effects, and high quality audio boxes, then closing and restarting the game; the boxes were selected upon restart. You probably need to save the game to get the options to "stick".
I believe that the U.P. has the games refresh rate set to 90 fps max? Could it be that you're using a monitor with a refresh rate higher than 90 hz, with V-sync applied/locked through Nvidia Control Panel? (Manage 3D settings/ Program settings).
