it is not. vortex trys to be very noninvasive in its technique. as you mentioned, it stores the files remotely, then it backs up the local files to be "replaced" and then it adds bizzare short cuts of the mods files in their place...
this is what ive observed vortex trying to affect. however, it dosnt seem to be working for me. or for my simple mod... its just txt files and a few audio files.
my game isnt recognizing these shortcuts vortex is placing in the folders though they are being placed in the correct places. so when the game runs and trys to load these files they all return "missing yada yada" and so on.
any one have any ideas? i tried running it in admin mode... and vortex literally scolded me for doing so... seriously.
it says its not meant to be run wtih admin privileges... yet when i was installing the mod, it kept asking for admin privielges to "elevate" the mods install... because it had to create the backups and add files(and it dosnt actually tell you this... it just says it needs to "elevate")... so... yeah. pretty stupid.
and of course most people run admin accounts as their normal user account...
so... not sure what to do... id have to log into my admin account to just access it while its running with full priv.
running it on a sub user account with admin priv disallows it from interacting with my sub user account files... yeah....... ie, it wont let me add files directly to it... and when i try uploading from the site... it opens a new sub user instance... wtf.

Are you attempting to only the one mod, or are you you also running the U.P.?
If running the U.P., is it running through vortex?
Which of your mods are you trying to run, the diablerie mod?
the unofficial patch is not designed to be run through vortex. i installed it the old fashioned wizard way. though i did toy with the idea of taking the extracted files and running them back through vortex(just for fun...).
it took some effort to get the vortex launched instance to run vtmb with upp. and im not really sure how i did it. it may have been as simple as restarting vortex. however i also went to my admin account and added the "VTM Bloodlines Unofficial Patch" short cut to my normal sub user desktop.
rofl noooo... not my diablerie mod... i decided to keep my test as simple as possible. my diablerie mod just barely works with a manual install... and then only just... XP
im just running my "kicking it old school" better guns mod. so... yeah. its just text files and 2 or 3 audio files. it looks like its all installed through vortex properly. eg when i go into the games main files, i can see the files vortex backed up and the shortcuts it added that theoretically link to the mods files.
but as i noted, the properties(the modded files) dont load into the game. so my gun inventory is empty on character load and when i try and console up a gun i get a "missing yada yada" pop up instead of a "glock 17 added or whatever" pop up.
what im thinking, now that ive calmed down and gotten some sleep...... is that the way vortex is operating is too intricate and exacting for it to be a dud. whoever wrote the vtmb handler files knew what they where doing.
infact, my kickin it old school mod was already properly formatted for the vtmb vortex handlers use... what are the odds? ; P
i bet if i installed and ran everything(steam, vtmb, the upp and vortex) while logged into my admin account, it would work. sigh. but if thats the case, i will never know. i don't run just anything from my admin account. especially not silly mod handlers. rofl.... just reformatted...
re-textured npc (I've seen the nines, damsel, Maximillion,& Lacroix, and a few others i haven't seen their changes yet,
nines now wears a jean jacket, Lacroix wears a blue striped suit, Dansel is more curvy(a guy thing i guess...)

I mostly chose mods for cosmetic types changes, i also have re-textures of the furniture & artworks(which I've only seen the ones in the asylum's club.
it looks like they changed the chance to save both sides of the sister(i know to be polite to both, and i did that,
unless sending a club patron to the blood bank for Vandel;
causes trouble i only found that quest recently maybe that has something to do with it i'll test that out, maybe i should send patty there instead if that's a better choice
That's good, I wish that I could be more help with the quest, but I don't actually "play" the game often enough.
One thing you could do to maybe satisfy your curiosity, would be to copy the necessary dialog files from the game, and paste copies in a folder on your desktop. Then, open them with the "bloodlines dialog editor" (if it still works), and find the relevant conversation, in the relevant file, to determine whether you had the necessary stats (the editor should show the required skills/stats needed) to receive the specific dialog "trees".

So, I installed the Kickin' it old school RPG version mod through Vortex into a vanilla (no other patches/mods) VTMB steam install.
Installing the mod through Vortex created an "Unofficial_Patch" folder (in the main game folder), which includes the files/folders from the mod only.
It also appears to have changed the timestamp on my vampire folder, to match that of the Unofficial_Patch folder, but the files/folders within the Vampire folder, seem to all have yesterdays date...
EDIT:
I downloaded the unofficial patch through vortex. When clicking the "install" button in the vortex menu, it opens a window that states "Vortex is designed to install mods from archives, but this doesn't look like one. Do you want to create a mod containing just this file?"
(shows the Vtmbup_104.exe).
Maybe nobody is actually trying to run the UP through vortex, which could explain why the UP nexus page comment section doesn't seem to have any vortex related complaints.
EDIT 2:
Installed the unofficial patch plus manually, tried launching the game through vortex, gameplay options menu shows "game mod loader", no "skip intro" option.
EDIT 3:
Set Steam library launch options ( -game unofficial_patch) for the UP, launched through Vortex; the patch appears to be working normally this way.
EDIT 4:
Enabled KIOS RPG mod in vortex, appears to be working (launched game through vortex, skipped tutorial, used impulse 101 command).
EDIT 5:
Saved the game with the UP and KIOS running. Shutdown the game and disabled KIOS, launched game through vortex and loaded save: appears to work (vanilla guns). Enabled KIOS and launched the game through vortex again, loaded save game: nothing appears to be broken.
So, mods appear to "currently" be working through Vortex if the Unofficial Patch (10.5_rc4)has been installed manually and modified launch options ( -game Unofficial_Patch) have been set in Steam Library - Vampire the Masquerade Bloodlines - Properties menu (general tab), have been set.

Maybe you could try using the "shared" mode (settings-vortex tab), which is supposed to use the same vortex settings for all users of the your machine. Or, maybe the "symlinks" option which claims to utilize some sort of admin privileges (settings- mods tab- "deployment method").
so kios was working? the easiest way i check is the 38's inventory name.

btw, where you running this all through a native administrative account?

like rip mentioned way back when, nexus adds the vortex button for all the mods whether or not they are properly formatted.
i did a quick test on another mod of mine and even though it wasnt in the proper format, vortex rearranged some of the files so that it would theoretically work. however some files in a sub folder labeled "optional models" did not get reordered. odd.
if you want to reformat a file so it runs with vortex, treat the main mod folder as the directory. so if i wanted to make a model mod, i would just put the model folder into the main mod folder. then pack it up with any of the approved nexus compression formats. i like 7zip.

installed the up10.5 via wizard
double checked vtmb's steam launch ops to make sure is still "-game Unofficial_Patch"
launched vortex; linked my account and browser to vortex. copied link vortex gave me and pasted the adress into my browsers. clicked authorised.
went back to vortex, selected games, added vtmb from the unmanaged list to my managed list.
went back to browser, went to nexus kickin it old school rpg page, clicked the download vortex button. selected slow... allowed url:nxm... im using chrome btw...
went back to vortex, selected mods, clicked install. then clicked enable.
launched vtmb from vortex.... and... rofl... it works... everything. the unofficial patch... kios... wtf.
no idea why it worked so smoothely and perfectly this time around. i didnt do anything extra or special... sigh. well i guess its a good thing.
edit: note, it never asked me to "elevate" during the install/enabling of kios like it did during my first test i mentioned earlier... just as well.
edit edit: so i went through and updated some of my mods with a properly formatted vortex file. it handled my model based mods and even a complicated mod like my survival mod with ease. let the good times roll...
still have no idea how i would implement my diablerie mod through vortex though... sigh. with finesse that is... maybe split it up into optional files... still so clumsy.
I only use one account, which claims that I'm the administrator. I guess that you use a non admin account as a "safe mode" of sorts?
I don't think that I actually saw all of the guns, because after using the impulse 101 command from the Santa Monica haven, I only receive the same six or seven guns (with or without KIOS enabled). But you guns had proper names and text and firing modes.
In the vortex -settings -download tab, there is written something about a "chrome fix" for Nexus mod links. I don't know if it's relevant.
I believe that one of the Gopher videos demonstrated using custom installers (with options), so it should be possible to create those.
