can't save heather
I've talked to knox, I repeat, I HAVE TALKED TO KNOX, but when I talk to her a second time after talking to the nurse it still just has her die, I have clan quest 4.0 installed and I think it could be that but previous versions have never been a problem

There's a question you have to ask when you first meet Mercurio in his apartment. Something about vampire blood. He first tells you about gouls. Then you find Knox, and you should be able to save Heather.
Follow this sequence that opens up the option to save her
1. Talk to Mercurio and ask him about the vamp blood
2. Talk to Knox and get info about ghouls and how they are made
3. Speak to heather and agree to help her find a doctor.
4. Talk to Malcom ( the doctor ) and tell him about heather
5. Return to heather and you have the option to feed her your blood thereby saving her

In the unofficial patch (plus) you don't have to talk to both mecurio and knox, I just talk to knox and get him to say something like like "and now I heal up real quick man oh yeah!" when he describes what it is like to be a ghoul ,then talk to heather, then malcom the doctor and then heather again where I am always able to save her. If this is changed in Clan Quest mod, then it is changed but I thought I should just put that out there.

guys I know how to save heather, I've done it before, I'm doing what I've done in every other playthrough, and I even tried talking to mercurio twice just to see if the game glitched out but no, noi matter what I do she dies

You also say in your original post you talked to the nurse instead of the doctor, which may have been a typo but likely led to the confusion. I don't use clan quest after the changes in 4.0 so I can't say further.

it's a typo, the guy with dreadlocks is a nurse not a doctor, he's wearing a nurses outfit

Uh, no, those are surgical scrubs and yes nurses do wear them but so too do doctors doing surgery and as Malcom says the guy he is working on has a bullet in his head. Besides the computers in the hospital confirm he is a doctor.

oh, I never actually read through his terminal in my previous playthroughs so I just assumed he was a nurse

Well it is the terminal on the other side of the door that leads to the morgue that says that, Malcom's terminal has emails you can read to blackmail him about the affair he is having.

But yeah, those clothes are designed with the idea of being fouled and stained in mind, which is why a lot of nurses wear them; they get the kind of work a full fledged doctor doesn't want to do like changing bed pans. They can be taken off and put on quickly so they are ideal for dealing with human waste, whether that is spilled blood and such during surgery or ... other things.

I don't think its about this. You need to learn about ghouls from EITHER Knox OR Mercurio.

ps. Malcolm ( the guy with dreadlocks) is a doctor I believe. I think you need to talk to him before you can save Heather? (He is unwilling to help and just leaves the PC to deal with Heather. Thats when you can do it)

You don't need to talk to Malcolm, at least not in the UP. But you need to ask Knox what a ghoul is, it's not enough to know that he is one...

Did you run the 4.0.1 patcher? According to Bergermeister, it is safe to continue a game after running the 4.0.1 patcher (last time that I checked the Moddb comments).

Is it possible that you forgot to launch the mod from the shortcut or modified Steam launch options? Just once is enough.
