Vampire: The Masquerade - Bloodlines

How scary is this game?
Is this game actually scary or is it one of those games that just has a general "horror aesthetic" to it and isn't actually scary but just has zombies and ♥♥♥♥. (Examples are games like DayZ which aren't scary [unless you're fighting a person] but carries the tag of "horror")

Hotel is a little scary but some people are not frightened at all.
When I had played this originally I had not been exposed to very much horror at the time. So, I was frightened at the hotel and maybe three other locations which I don't want to spoil for others, The natural park with the werewolf, briefly with the flesh eating vampire in the abandoned hospital, investigation into the hollywood Tziminsce. And there may have been more moments but those were most memorable for me.

It depends how immered you are. When i'm fully immersed this game still creeps me out sometimes. Especially certain parts when you start thinking about them.

The early Ocean House Hotel was scary my first time through. Played in the dark, headphones, not knowing what to expect at all and also not having played many horror games before this, or at least haven't played them in years.

After that, there's some pretty creepy stuff. Unnerving. I won't spoil anything but try not to read any spoilers of what to expect. There's just a lot of atmospheric creepy stuff and as Vepar said, stuff that is creepy as hell when you stop and think about it. Not really a lot of jump scares or stuff like that overall, though, it's all ambiance.

It's not scary, instead, it's some really nice game with RPG elements, the race you choose changes the entire dynamism of the game and choices that greatly affect the history. Ocean Hotel and walking through the sewer can be a bit scary though, it really depends on how immerse you are.
I don't think it's an horror game, they maybe 've put some of this but it's not a feature of this game. It's based on a role-playing table game book after all, so you can have some fun evolving and involving yourself deeply in the story.

The only really scary part of the game is the haunted hotel.

RPG Elements? Rofl. This is a true RPG, more than the crap these days, table top based. Witcher 3 have RPG Elements but not this

Yeah, and they jump out at you and move fast. I hated those things, too.

This game isn't scary at all. The Ocean House quest might be a little spooky for first time players, but that's it.

I Never found this game scary. It has a nice creepy atmosphere a lot of the time, but not really scary.
Even playing the ocean house quest for the first time was a cakewalk for a very simple reason; You play a f-ing Vampire! You ARE the thing that goes bump in the night. YOU are the boogieman. You are a nearly immortal undead bloodsucking monster. What the hell do you have to be scared of?

Well when that first chandelier fell on my health and nearly killed me, that kinda triggered the "this ♥♥♥♥ is not a walk in the park" response, so whenever something moved, i kinda flinched. It's quite ingenious honestly. Yeah, you're a vampire and you go BOO to everyone else, but there's still stuff that can hurt you. Even from the "harmless" ghost. :)

I think elements of it are pretty unsettling. I feel like the bad early 2000s graphics play a part in making it more creepy for some reason.

I found the music in some quests/areas is pretty creepy.

I agree with Skulldogx that A Plague for the Angels and Going the Way of Kings quests were pretty scary. I hate doing those quests because the thought of it just creeps me out. Weird culty ♥♥♥♥ and snuff films... no thanks
