I would agree with this opinion. I would also not lock under any mode. I think the Custom Mode should be accessible to everyone instead of hardcore players. While Normal mode is easier now, it would still severely limit the amount of players who could try this.
And regarding what extra rewards could be for beating Nightmare mode: not precisely sure but it should not be gameplay-wise. Excluding the new ending and Steam achievement, I would probably add a small environment asset for any future gameplays (rough examples: a trophy or confetti). In other words, something simple that does not require any animations or logic to add.
