To equip an infusion, just use it, it will either add it to your skill list automatically, or prompt you to replace an already equipped one.
The Demonic soul is just a free starting time for everyone for an Expansion's aniversery, it doesn't have to be equipped, if you read, it says it has an effect "When Carried", that means its a passive item that gives its effects as long as its in its inventory.
Ego just means the weapon has bonus attributes.
(Arcane) Disrupt(ing Forces): the first time you equip, there should pop up and tell you about Magic v Nature. Basically, the item is anti-magic, it has an effect that is effective against spellcasters, and it can cause your own magic to fizzle if you try to use any magic while the item is equipped.
The Tome wiki is very helpful for understanding what game mechanics do, though it is a little out of date (1.6 just came out).

For your questions:
You start with three infusions or runes equipped. On you hotbar at the bottom they would show up as a shield, a short blink (teleport), and a shatter inflictions effect, which gets you out of trouble if you are stunned or on fire or something. Or, you may have a heal, regen, and wild infusion. It depends on the class you are playing. If you picked up an additional infusion you can choose to replace one of those with the new one. Or, if you make it to level 10, you can use a category point to unlock a new slot.
The demonic soul is something you can equip on an item. Different souls will have different slots they can go in (like one soul can be equipped to your primary weapon, a different soul to your armor, or ring, etc.). You unlock the ability to equip these to your pieces of equipment through one of your skills, the "Bind Demon" skill. When you have a soul you want to link on a piece of equipment just drag it from the list in your inventory to the piece of equipment you have on.
Tome is really complicated, I agree. But there are lots of people who are happy to help on this forum. Or you can try asking questions in chat in the game. I hope you enjoy the game, it is very challenging, but also a lot of fun.
This will make anything that in its description says it "Is: A Spell" have a chance of failing.
When you attempt to use/cast the talent it will have a % chance of failing, but taking up as much of the turn as the talent normally would anyway. Also If you have a sustain that is magic active, there is, IIRC, a % chance per turn it will de-activate. I've accidentally equipped such items wondered why the heck my sustains kept auto turning off randomly while resting.
Edit: More about infusions:
Once "equipped" you cannot un-equip them. you can only "overwrite" them with new ones. Also keep in mind you can only have 2 of the same exact type of infusions/runes equipped at a time.
So you could have 2 Healing infusions, but you could also have up to 2 Regen infusions if you wanted. Or have 2 Shielding Runes, but if you get great RNG and find 2 Mirror Shield(?I forget the exact name) Runes you could also have those. They're pretty rare so don't expect to come across them in every game, or even every few games. The mirror ones are damage absorbing shields too, but they reflect damage back at the attacker.
That's a Demon Seed used by the Demonologist class which is part of the Ashes of Urh'Rok DLC and must be unlocked. The Demonic Soul is, as stated above, a special item given to people to celebrate the anniversary of said Ashes DLC being released.
