Achievement Bug - Unfinished Story (workaround)
So upon my initial playthrough, I looked in each nook and cranny. I found the grenade, eventually coming to a part where I could use it. After the detonation I noticed it would be some difference maybe downhill. Later into the story after getting to the dam control, I was looking around where to go next. As I proceeded to the next area, I saw a scuba diver with a sniper rifle next to him. I wondered if that were actually it. There was no interaction available, no achievement, but the Scuba diver was there with the sniper rifle.

I exited the game soon after for a short break (since I just had a save in the dam control room nearby).

I load back up and it has me run through that area again but this time I notice nothing is there. No Scuba diver, etc.. So I backtracked to the part where I threw the grenade but the prompt was not there. I then went back to search for where I picked up the grenade and there it was again. I picked it up, continued back towards the bridge, the option to throw the grenade again was there, and so I went back to look if that Scuba diver would reappear.. and YEP. The scuba diver was not only there but I could interact with the sniper rifle. Upon doing so, the achievement popped and I continued with the story until completion.

Hopefully this helps any others who encounter this bug.

Thank you! Had the same problem and I was doing this playthrough mostly for that achievement. It feels like there might have been a time limit or something, but I don't know. I solved all the puzzles fairly quickly I thought, though I guess I could have skipped the portal house and the sea-thing to speed it up a bit.

Omg, thank you very much! First diver wasn't where must be. So I just go back and find grenade again, throw again and diver was on his place after this! You saved me :)
