(Spoiler) Mask doesn't stay equipped

I have found a mango and have equipped it in my passive slot, giving me access to the mango mask. I equipped this immediately. It worked as intended for about 30 minutes but now whenever I rest or get knocked out of my shell, the mango mask unequips. I can't get it to stay equipped so my Harbinger now never gets to actually wear it. I am playing on Xbox Series X but this was the only place I found to report bugs for the full release. Thank you.
