What is "Dice with Death"?

I get this every time in "Deal with the Devil" - after a couple of tries (over 100 turns each) I never find the quest/encounter. Is it bugged or do you have to do something particular in order to trigger it?

Dice with death is a field in the inner circle

When u entered the portal to the innermost region u go right (the path of strength) overcome the crypts and then u land on that square. It is easy spotted since it has a Grim Reaper on it.
Roll first (high) and then Death rolls, if u are higher u win.
