You can tell how many shards you have from each area by looking at the center platform in the map- you'll notice each cardinal direction has 4 dots- they will illuminate pink whenever you've collected a shard, so if any remain greyed out it means you're missing however many shards from that area.
so collect 3, enter the door that needs 3, kill the boss, activate the big thing after the boss
rinse and repeat in the remaining areas
No, you need four. It's indicated pretty clearly on the map and on the PC located in the Drifter's home within the hub town.
You seem to be operating under the understanding I'm referring to area bosses. The question of this post was how to reach the final boss (Judgement). I apologize if I wasn't more clear, but I would also recommend you double check the title to a discussion a couple times in order to avoid misunderstandings.

Hey no problem! It's easy enough to misremember things! I have to be honest, when you first mentioned needing only three I went and quickly loaded up my game just to be doubly sure I wasn't wrong myself. :)
