Haha, you're not going to do it on New Game+. You die in one hit from half of the enemies...
I recommend doing NG+ before you go for no-death because it forces you to learn the bosses very well to deal with their 2-damage abilities (and you'll get used to having a tiny health pool)

So, the game doesn't count deaths, exactly. It counts hard resets (the red distortion effect) that you see on waking up. This effect looks like you've been knocked to the ground and you're struggling to get up, and also has the red filter jittering over it. That's the count.
There are two ways you receive the count, not just one. The first is, of course, when you die. That's the no brainer. The second, is when you hard quit the program. This is important to note, because if you want to maybe take a day's break between the south boss and the final boss, and you simply quit out by hitting the X in the window, that's a "death". When you log back in, it will play the "Dead recovery" effect, and then you'll continue on.
This means two things: one, you can't just quit if you do die, and you want to "beat the clock" and erase that mess up. Fair enough, just gotta get good. But two, is that unless you've been really diligent about exiting the game through the menus, and also quitting the title menu through the prompted menu buttons (don't just quit the window while in the title menu), when you load up your next save, you're going to wind up with an unaccounted death.
Don't let what happened to me happen to you. Save lives. Be informed.

Eat my ass. Knowing how a game calculates this kind of thing is important for this exact reason.
I'm kinda fuzzy right now. So, to "properly" quit the game without getting an unwanted "death", you have to pull up the in-game menu, quit to title screen, then click on "exit game"?
Thank you for posting your findings dude... I was wondering if I had to play the entire game in one sitting or would I be able to exit the menus and save progress... I guess the former is the answer to my question. Will be a tough task, but I'll definitely give it a try.
Oh and FYI: Don't let trolls get to you dude... You'll always find one lurking among the crowd. Better to just ignore them. They can't stand being ignored lol!

No. Why would you have to "exit game"?
You can save and quit... Just dont die...
Some people don't quit properly, they just exit the game. (Don't ask me why, but it's a thing) There's also the question of whether they can quit during a boss fight to avoid dying. The thread is reasonable.
