You could argue that nobody shoots perfectly where they're aiming, but I think the actual blame goes to the projectile hitbox and hit detection algoritms being different between the aiming system and the actual projectile.
It does often surprize me when trying to do some very precise trick shots to hit enemies or switches between pillars and walls, confirming that i should be hitting my target with the laser sight but mysteriously hit the wall.
I did manage to get some screenshots of it happening, though.

As you can see, depending on the angle, the actual trajectory deviates variably from the predicted one, with the last picture being a close example of the most extreme case. This makes trying to hit things from across the screen with the pistol very difficult and unpredictable.
I should also mention that the pistol is the only gun with which I've noticed this happening, though I don't have every gun yet.
