When you arrive in the graveyard district, do not use any of the three entrances that you used back in chapter 2 to access the lower tombs for Kagain and/or Edwin's quests or chapter 3 for your first confrontation in her lair.
Instead go all the way to the last walkway and turn left. There will be the area where you pulled out the guy who was buried alive. To the left of that will be a small crypt where the Crypt King is. To the left of that is the new entrance to Bodhi's lair. She has sealed her lair off from the other 3 entrances. Even though that was how you entered the lair the first time in chapter 3, you have to use this new entrance for the final confrontation.
