Also, four medical centers will probably have less traffic congestion since they are spread out compared to the one hospital.
Since this is a simulation, citizens that are sick but don't need an ambulance don't show up although they would be represented by some of the vehicles you see stopping at the hospital or medical center.
And the Floristry shops/industry :)
First thing that comes to mind is sewage. Where is your water pump in relation to your outflow pipe? If poopy water isn't the cause then check your pollution areas. Is purple grass near any residential zones or water towers?
Back to your problem: assuming your hospitals are nicely spread out and no traffic jams hindering your ambulances and assuming aswell your health budget is above 75%, the reason is most probably pollution.
People check ground pollution pretty often, but you might have forgotten to check noise pollution. And like someone said: make 100% sure your water entrance is clean.
Hint: when you open info on a building with sick people, hover the mouse over the sick icon on the left and it will tell you why this persons are sick.
Using only the wanilla for the exception of a few parks and intersections, I have to say I tend to have no issues at all with my game experiance. If something goes wrong it's all about me and nothing else.
