Its strange, Unlock Every Item does just that but Infinite Money unlocks everything and gives all items.
Not sure if that's by design or not?
No, you access from the main menu, think its tools > MIDs from memory but I might be wrong.

Can you only choose one at a time? Are you sure you didn't have them both on by accident?
In the game menu click content manager and under mods there are 3 mods that installed with the game... press on and start a new game
Edit: I answered too late :D :P
Hard mode makes all the values of the game (costs, how long it takes for stuff to level up, how much you cash make etc) harder, I watched a video and most of the values were around 20% more difficult than standard difficulty.
It moddable though so in theory Super Hard more or even Super Easy mode mods are possible.
