Why do everyone suddenly get sick?
I can't figure it out. Everything is running fine according to what the game tells me then BAM! Thousands of sick people and my city dies in a matter of days.
Water and sewage is in the green. Garbage is in the green. Law enforcement, everything. Why does this keep happening? It's like I'm triggering some anti-piracy thing.

This is the sixth city that dies a few weeks after crossing 16K population.

Please post a screenshot of your city with the pollution overlay on.

This one? The game isn't really forthcoming with how to get information.

Are your water intakes downstream of the sewage outflow pipes?

No. Miles apart.

On your info panel that drops down from the top left icon, there's a purple oil barrel thing that should show pollution. Any pollution including noise can cause illness, but water pollution is a particular killer. If those are looking good too, then I'd say you're experiencing your first deathwave for the city. Hit the info icon left of centre on the main hotbar bottom of screen. It's possible your first settlers got old and are dying off. If at any point you have more senior citizens than adults, you're probably in for some widespread deaths.

FFS, the game constantly spams you with basic crap but that REALLY IMPORTANT thing is just completely ignored?
Fine. Whatever. They could have at least pointed it out it was there. -_- Keeps talking about overlays and I assumed it was hidden somewhere on the panel.

16000 populatoin in 4 game years = death wave , l think

Oh, if you mean that birdy thing, I couldn't agree more lol. I think there's a way to make it shut up.
Honestly, I'd say you're weathering your first deathwave for that city. The only way to avoid or minimise them is to be careful not to open up too much new residential zoning at a time. Otherwise a whole whack of new citizens move in and grow old and some years later they all die off.

The only thing that can actually kill your whole city in a matter of weeks is drinking water pollution. If all of your cities have been on that map (Diamond Coast), I can easily see that water pollution eventually creep up to your water pumping stations and wreak havoc.

It isn't as the outlet is at the south end of the opening tile and the intake is at the north end.

I'm familiar with the map and how vulnerable that spot is. As you can see at the bottom of the pollution screenshot, the sewage from drain pipes remains lingering on the spot because the current isn't strong enough to wash all of it further downstream. The water pumps locally reverse the weak current and eventually suck in the piling sewage.

Thank you, that's the one.
The good news is I see water pollution is 0%. That's good, keep it that way.
Water pollution is what would cause citywide sickness and death and even 1% is enough to do so.
Still, from the screenshot, I don't see masses of sickness or death icons and your population is actually increasing. So, where is this disaster?
If there are any sickness icons, click that property to open it and view the warning message by hovering your mouse over it. That will give you more information, such as the reason for sickness. When you know that, you'll be able to work on solving that issue. It will likely be due to ground or noise pollution affecting the residential there. That will make them sick, but shouldn't cause death. It's water pollution that is fatal.
Here's something else to consider which may help to explain what others above have noted.

It's happened so often I don't bother to save very often. I'm about 2-3 months away from the event itself. Also: Thank you for point out more ways to get useful information. Wish the game did that.

Oh ♥♥♥♥♥♥♥♥♥♥♥, you're right. How did I miss that?! D:

on some maps the water towers are just a better choice than the water pumps.
