Often they started out as a village in medieval times. Like at a road cross. Or near/around a castle. Often a lot of places started as a roman castel/fortification and became modern settlements over the course of time. Or some fruitful special place in a valley near ressources ( farm land, water, mining, military strategic point) and so the road connection came. If the foundation was fruitful enough, it continued to grow into a bigger city into modern times. With changes, of course.
If it has a castle sooner or later someone would have bulldozed some royal avenues through it.
My advice? Start as small villages, and let them grow. Don't be afraid to do the changes the ever growing city has needs for, and you pretty much and up with a more european style.
