Learn why so many are dying and see what you can do about the cause. Or more importantly, be aware of how much you may or may not be responsible.
So, first you have to rule out the chance that so many dying is not due to unnatural causes such as water pollution. View your pollution overlay. Water pollution must be at 0% at all times for a healthy city. Just 1% is enough to cause citywide sickness and deaths.
If the deaths are due to natural causes, then that is a natural lifecycle of your population and the percentage of your population that are seniors, usually around 24%, is what you should provide sufficient deathcare services to handle.
You mention you have both crematoriums and cemeteries.
I would recommend you empty and get rid of the cemeteries as soon as you unlock crematoriums. You may keep a cemetery or two for the sake of appearances if you wish, but in that case just turn them off.
You also need to consider whether the seniors are dying en masse at one time, or is the rate of death even and steady. The way in which you zoned residential will be a significant influential factor. For more on that see here,

Lastly, consider your traffic management. Hearses have to be able to reach all buildings that require their services and they have to be able to do that on time in order to do so efficiently. So, if you have any road congestion that may hinder them, then that needs to be resolved.

There is a mod named District Services 3.0 in the workshop. With this mod, service vehicles will only service the district they are located in. I haven't used it, so I can't say if it's worthwhile or not.
