Please post a screenshot of your game with the pollution overlay on. If your water is clean, then water pollution will be 0%. If it is anything higher than 0%, you are poisoning your city.
hover over the icon in the sick house hold, it will tell you why they are sick.
The "main" one with only the thermometer
The one with ear protection (sound)
One look just like the "main" one but with a tiny pail of water @ bottom right ( water) lol
And the last one is land pollution, it looks like the water one but with purple pollution thingy at the same bottom right.
When they get sick they do tell you from what, though when the need for medical assistance becomes greater the icon will change to the "main" one.
So if you “Ever” see the “Sick by Water” icon, Pause in hunt the source with the pollution overlay.
The three forms of pollution all cause sickness, however, water pollution is the only one that is fatal. Nobody dies from ground or noise pollution, even if they have no access to any health service, but they will die from water pollution.

I asked myself the question, “ why would only water kill”
