Citizens are sick for no reason
My citizens are sick and dying eve though there are lots of hospitals and medical clinics nearby. I have put parks nearby as well but that wont work.

you're poisoning the drinking water. you either have water towers in your industrial zones or you have put a drinking water intake near to a sewage outtake

The drinking water is safe. There is dam seperating the polluted water and the safe water.

Heh... prove it.
Please post a screenshot of your game with the pollution overlay on. If your water is clean, then water pollution will be 0%. If it is anything higher than 0%, you are poisoning your city.

As Grapple just said, "prove it". cims only get sick from pollution and only 1 type of pollution kills - water. This is fact, there are no if, buts or maybes.

It could also be noise pollution. My people always get sick if there is noise pollution.
hover over the icon in the sick house hold, it will tell you why they are sick.

But noise-sickness has a different icon and does not kill anybody. Beside of this, if your people get sick from noise, they start to complain about all the noises on chirper.

Look closer at the sick icon itself when it appears over a building to notify you of issue. There are four different sick icon.
The "main" one with only the thermometer
The one with ear protection (sound)
One look just like the "main" one but with a tiny pail of water @ bottom right ( water) lol
And the last one is land pollution, it looks like the water one but with purple pollution thingy at the same bottom right.
When they get sick they do tell you from what, though when the need for medical assistance becomes greater the icon will change to the "main" one.

No. Only water pollution kills.

Sickness does not kill. It is when the sick do not get picked up in time that kills. Therefor all sickness will kill if they are not picked up! The reason it seems that only water kills is due to the area affected. Noise and land pollution are in small pockets so to speak. Though water pollution hit a very very large area and possibly spreads. The issue is the sick cannot get picked up in time simply based upon the sheer volume of them. That’s what kills.

So if you “Ever” see the “Sick by Water” icon, Pause in hunt the source with the pollution overlay.

I'm sorry, but no.
The three forms of pollution all cause sickness, however, water pollution is the only one that is fatal. Nobody dies from ground or noise pollution, even if they have no access to any health service, but they will die from water pollution.

There aren't any "pollution filters" in a dam. You better look at it more closely.

From personal experience it is quite possible to be picking up a faint trace, like 1% water pollution, in an intake, and you would not be able to tell by looking at the overlay. The only indication that something is wrong would be anything beyond 0% in the water pollution statistic.

I know it's been like 6 years, but I also had the same issue and I found out it's cause I accidentally placed a water pump where I put all my sewage outflow without realizing.
