They would have to have an identical set of mods and assets that you used in the save game.
There are many different reasons why a developer would avoid making such a game. Chiefly, it would be the question of how you would set up a server or require players to host their own server. It's too much of a headache and unprofitable for the dev studio to run one for players. You only have to recall how EA failed, failed and failed again with attempting that with SC 2013.
Despite the practical delivery of such a concept, imo the problem lies with the players themselves. It would be very difficult to find somebody to co-op with who was your equal in terms of skill, talent and shared your build ideas or plans. Let alone somebody who had the exact same set of custom content.
For example, when playing Tropico in multiplayer, naturally they are your opponent and the best way to beat them is to outbuild them. You're both building your own Tropico cities on the same map but competitively and not co-operatively. When two players build their cities that share the same map, it will always be so.
So, the option I suggested above, to play the same game on a turn by turn basis is the only way to co-op and it's not a bad way either. Have fun trying it ;)
Was talking to a guy at work about that today. He said the available building size for multiplayer was too small for one person, much less a group of people.
Hey man, OpenTTD its has older graphics but it is addicting and multiplayer. I like the SoraPack edition of it which gives it a few cooler things and I think better multiplayer. I would give it a chance. https://github.com/koreapyj/openttd-1.5.3/releases/tag/1612304
