Unable to obtain achievements

I cleared all levels S but couldn't get any achievements.

Are you sure you've beaten Puppet Maiden? Her portal does not spawn until you beat everyone else.

Also, I had situation once when I had to beat Niten Usagi once more after rebooting game to fight PM again.

When you spawn at the start and turn around, do you see a critter with "Thank you for playing" message and list of all your best results?

The only solution I can think of right now, is to re-beat all opponents again/reset save and do it all over again.
