Alarms only go off when an alarmed door is opened, a window is broken, or C4 is detonated, when the alarms are still active.

Cops are called:
When the alarm goes off,
cameras see a body, masked player, or tied up civilian,
an unsuppressed gun is fired (guards or player),
a pager is not answered,
if guards find a body/tied up civilian,
and sometimes if civilians see you with a mask and run (but not always).
