Check out this flamethrower guide. This is a great channel too. Lots of good TLOU2 tips.
It's incredibly useful in the areas just after you get it. It's not an insta kill but it does damage over time and stuns enemies so you can get some distance.
I noticed that if I held the trigger down it didn't seem to do much, I'd run out of ammo super quick and they'd still be alive. However as nuclearaddict said, if you just tap it and hit them with a blast, it stuns them and sets them on fire. By the time they start moving again the fire is starting to go out and you can blast/stun them again.
To be fair to the flamethrower, Abby's half of the game is brutal on Grounded. Everything felt like it missed and I never had any supplies. I loved it!!
I haven't tried grounded yet actually.
This is the third time i have played the game though and this time i finally realised that as Abby you aren't supposed to stealth as much - it's easier if you are way more aggressive and then fall back to hide if it gets overwhelming. Not sure if that still works on grounded.
It's a you problem.
