There's a guide for it up. Long and short is, get your Karma up high, then get headpatted by a demon with a heart mark on the field sprite (there's one in the Sarian Ruins) and carry it to term. You should get the option to transform if you've met the requirements. Haven't tried it myself yet, though.

if you are trying to do the evil route in general, your karma needs to be at a certain point near end game to follow through with it. so keep raising it.

I had karma at 100 and a bit, that was enough. So I assume 100 each should probably be sufficient.
