Did you get stuck in hole? I've only had it happen once. I usually go with Destruction Amulet and pistol or summon block. Sadly, you may be boned. Is that cloak you have?

You have a 1% chance of that happening. (Based off my hundreds of runs) Next time you can start with a run through Halloween world (keep running to be invincible, and dodge cultists). I usually get lucky with a few health fountains.

After this you could run Dagon a few times to build up soul coins and bullets. Keep a good stash in you chest to abuse cloak with for a quick run. Sorry that happened, I know it was a real bummer for me.
