And something to test, as it may or may not help, is downpatching to an earlier version, if it still occurs there, then we'll have to think some more.
About 30 FPS, maybe less.
How do I revert to an older version and then back to the current version? I'd purely do it to test for you so I'd want to resume playing on the current version.
And to opt out of it and revert would pretty much be the same.

So I just tested it and it runs perfect in the older version in fullscreen.
I played a whole level to be sure.
I reverted back to the current version and FPS was low straight away - I even noticed it when rotating the level-select cube.
As for 1.2.1, this won't likely fix it, as you've kind of already reinstalled it switching from beta and back, but if you right click qbeh, properties, installed files, verify integrity of game files, it could help (may also be worth keeping a backup of your save, can never trust these things).
It would also be interesting to know if you have qbeh installed on your main internal drive, or on a secondary or external drive.
Do you have multiple monitors, if so, do you run it on the main or other ones?
Do you have other possible conflicting applications running alongside it?
Do any other games suffer similarly?
What happens if you launch it from the exe directly, not through steam?
Does the low fps occur in consistent spots in the game? is it specific levels? or parts within the levels?
Does it ever revert back to normal fps on its own without changing settings?
Are you using the in-game setting to make it fullscreen or a third-party software?
Are you using anything like borderless gaming?
Do you have any launch options set? (right click qbeh, properties, launch options)
This should be a few things to start with, hopefully with a bit of mashing we can find the cause.

Could try disabling steam overlay to test, steam settings, in game, untick.
Would also be good to know, is it a laptop or desktop? and do you have a secondary graphics card?
And what does task manager say when you get this lag? what looks to be struggling or too high?

Disabling Steam overlay didn't fix it.
It's definitely tied to fullscreen for some reason. I tested a bunvh of times by applying fullscreen, restarting the game and playing a level and FPS was low each time. Then I disabled fullscreen, restarted, played a level and FPS was smooth.
In both cases I restarted several times to be sure.
As for your questions, I'm on desktop and only have one graphics card.

Weird thing is that after taking the screenshot above, I went back into the game and it was running at 60FPS in fullscreen. After a restart, issue was back again.
I just tested running it from the game's executable again and FPS was low in fullscreen. So I'm not sure why it was fine when I tested that yesterday.
Is your monitor's native refresh rate 60hz or are you limiting it?
I don't think it should be there but if it is, could try to rename or delete OculusPlugin.dll in Steam\steamapps\common\Qbeh-1\Qbeh-1_Data\Plugins, people had issues with it in the past, but it's since been removed from the releases.
Ooh, what about alt + enter? it'll make it fullscreen, but it won't tick the box in the settings, may or may not have a different outcome.
Or what about trying something like borderless gaming?
And I don't know how much you want to test, but could make a new windows user account, only dl steam, login on that, reinstall, and test.
It's 60Hz.
That plugin isn't there.
Alt+Enter works but when I start another level the problem is back.
It also disabled the pause menu for some reason.
Not keen on making a new user account to download Steam and the game.
But I don't want to leave you with nothing, so it could be worth testing individual patches with downpatching in the less easy way (it's still straight forward):
