I made the same mistake as that other guy, fell down the far right hole after lighting the torches, left, came back and the siren was gone. "Crap, it's one of those one-shot encounters" I thought, but I had saved the game before I tried so it was no big deal, right? Except when I quit, reloaded my game, and went back in to try again, all the torches were already lit, so I couldn't summon the siren even though it was technically still my first time. I was also playing Vladyn if that matters, which it might; one or more event flags for the mausoleum/frozen crypt might be glitching out due to his altered early game. I'm going to go check it with the other characters to make sure before I post again.
Note that I have only one save slot in use, so I don't exactly know how the save names will differ with multiple, you'll have to figure that out for yourself.
