You need to make Lucille duchess of Merva. I can elaborate further obviously, but that's the barest bones. Or did you mean the stats?

It's the sick elodie window, but with an Intrigue score.

so basically you could call it making all the wrong descisions but living long enough to learn from them.

Late to respond, but you get this one not just by making Lucille a duchess, but also causing Joslyn to die and executing the whole Merva family. This may result in a rebellion on Week 39, but if you manage to avoid it or survive it during the epilogue Elodie will end up elevating minor lords and ladies to Caloris and Merva and getting this epilogue.

Also, despite what it may seem, appointing an administrator as ruler of Elath will NOT give this epilogue

@Piroco I think the rebellion is the "province in rebellion" acheivement which is it's own seperate window. This is the window with somone who appears to be getting knighted, not the tourches and pitchforks one.

I double checked just now and if you have sufficiently low Noble Approval you get this dialogue and the Royal Gifts epilogue:

The tragic events of the past year left a large gap in the Novan power structure, with both Caloris and Merva now lacking in leadership. As she had been closely related to both Dukes and their territories lay close to the capital city, Elodie chose to temporarily claim those lands for the crown and put them under her direct control. This was one step too far for the remaining nobility. Facing the prospect of universal revolt, the new Queen backed down and agreed to elevate minor lords and ladies to fill the vacant roles.
