After Netty gives a gun in the church, you run past the dogs (shoot until it's worth it, they'll eat it). Behind the church on the other side there is a fence and a closed cellar with a lock. You jump on 2 barrels, shoot at the dogs. When they all die, shoot the lock, open the door. And this is where you get into the water. If you can swim underwater, you get to the island. If it doesn't work, then you drown and he takes you to the dead zone. And here I just jumped into the water and he immediately switched me.

I had just the same problem. I don't know, if I fixed it, but it doesn't happened again. And I am not sure if it will help you. I started to play Shadow Man with DGVoodoo2, because of the a few additional settings, AA, 60 fps....
So as I went to that place you mentioned, at first everything went fine, I dived into the water to get the cadeaux and then I tried to get the other cadeaux and suddenly I died. I reloaded my savegame and tried again. This time I just touched the water and died immediately. I tried again and again and it happens everytime.
I tried everything to fix this, but I at the end I deleted the files of DGVoodoo2 and it seems to work. I swam through the water without any problems. I am not sure, if it is DGVoodoo or a setting I made. At least it works now. Did you play it with DGVoodoo aswell? If yes, then remove the files and try again.

Just play the remastered version, Don't expect an update for a 1999 game.
