If you are new to the serie, I would strongly advise to try the previous one to see if you like it. You would not notice most of the differences.
There are many subtle changes between the two games, so many that it would take ages to detail them all. However, nothing as drastic as 1UPT brough to the Civ franchise. But we have an all-new magic path, Glamour, as well as other new features. None of these have such a dramatic impact on the gameplay though.
There will obviously be some folks who are completing their previous Dom 5 games but there will be a few 'never-Dom 6' players as well. I have no idea how many players fall into these two categories but I'd imagine most will move to the new game eventually because it's got more to offer. If you want to play MP, Dom 6 is the future but Dom 5 will do for now.

Ingame diplomacy system and updates to the AI to use it.
Yes, it looks the same (quite better actually, some new sprites look very nice), plays the same on surface but if you dig in and you know previous game pretty well you will experience very big amount of changes. And we can see that the game will keep up with new nations and improvements until another 5-6 years pass and we will get to enjoy Dominions 7.
And apparently there was even more. They even made the list themselves, it's just isn't available in Steam. Would have been helpful.

Also I looked back and noticed Dom5 came out in fkin 2017. 7 ♥♥♥♥♥♥♥♥ years. Feels like it came out during covid
