5 nights at mom's achievement

How can I complete this achievement? I’ve gotten a streak of 3 wins with different characters and haven’t received the achievement. Does anyone know what I might be doing wrong or how to complete the achievement?

Depends on what DLCs you have. From what I've heard, the achievement bug was fixed in Repentance Plus. If you have just Repentance (not Plus), then read more here:

If it's really fixed, I don't know what could be happening.

It's called 5 nights for a reason, you have to get a 5 win streak with different characters (in Rep+)

if you are starting a new file in Rep+, the first run where you kill mom's foot does not count towards the achievement. Also, fun fact, if you pause and exit before that first cut scene start, you can continue the run after; however, even if you manage to continue past mom's foot (with we need to go deeper or a sac room to the dark room) the win still doesn't count. but you will have a 2 win streak if you finish the run. ;P I had to get a 7th win streak because of that.
