The Guillotine, I'd even go so far as to say it's the worst item in the entire game. As someone who plays T.Lost exclusively, I have more confidence in a run with Ipecac than Guillotine.

Blood and guts entirely due to a massive coding over sight with T. Eden
basicly if you ever get rerolled into it your gonna potentaly lose all your red hp without any way to stop the bleeding consistantly

if we are talking just wanna see it removed from the game level of hate, Im gonna agree with you on Bumbo can't tell you how often his warningless bomb dropping when collecting coins behaviour has forced me to take damage, ON TOP OF THAT why the hell can max teir bumbo drop coins? he instantly picks them back up

Epic Fetus.

Overrides all your funny tears, rivaling Mom's Knife in terms of how little synergies there are and existing one's are all super boring.
The delay is also awkward in combination with the way you fire/aim it. You also can't really increase the rate of fire, only make more missile's drop. By far the least fun item, that can win you the game. Unlike Mom's Knife, i won't even take this to salvage a bad run (and it still struggles to defeat the tankier final bosses - good luck vs. Beast without an absurd dmg-stat).

Clicker, its quite literally useless, bugged and pointless. If i wanted to unlock a specific item with a particular character, i would PLAY that character, not make a gamble with 1/34 odd

Firemind. I've lost more runs to that tear effect than I think just about any other. I think it's a rare enough item that if you get it then you should be immune to the explosive effect from it specifically.

As good as Tech X is, I find using it annoying. I'd like to skip it, but the fact that it's actually good makes doing that feel wrong.

Blood Oath, while good, more often than not causes me to lose runs. The fact that they made a somewhat frustrating challenge based around it proves how bad it is IMO.

I'd not expect a T.Lost player to dislike Guillotine when it's a orbital defence item, damage up and tear delay down. I'd even say BBF and Little Baggy can cripple a T.Lost run more effectively than having to aim slightly more dynamically with Guillotine. But it's all a matter of opinion.

For me, it's tied between IBS and Lil Portal.
Taking away my shots for a second to unload bombs or explosive gas in the middle of a fight is miserable and dangerous. A very weak damaging familiar that absorbs useful resources in exchange for flies and a portal that often gets in the way has only ever left me wishing I had Angry Fly instead.
