Like you said, it's a glitched item. It has random sprite, name, description and game effects.
After you beat Delirium as Tainted Eden, items in Secret room, Super Secret room and I AM ERROR room have a chance to be transformed into glitched items.
If you want to see item's effects before picking it up, install External Item Descriptions and put "--luadebug" into launch parameters.

thank you very much for the answer. one more question, when this happened for the first time, I got a glitchy item after killing “Conquest”, instead of his white horse I got a glitchy active item. Another way of generating glitched items is to pick up TMTRAINER (Tainted Eden to Beast). Though, if you pick it up, all further items will become glitched, and it's not easy to get rid of it.
Maybe, you got TMTRAINER as a wisp from Lemegeton (Tainted Bethany to Delirium), transformed White Pony to a glitched item, got rid of the wisp and continued your run as normal.
Or maybe, you got lucky with rerolls.
Can't say for certain, unless you provide a recording or seed.

EDIT: not 100% sure if Lemegeton can give you items you didn't unlock. Just pure speculation.

Surely this could happen. This book turns out to be able to create items that have not yet been discovered? unfortunately I don’t have the seed and the recording, but thank you very much
