Sharp Plug Breaking After R Key?

Just had the strangest thing happen. I'm no stranger to this games jank but this is something entirely new to me.

I had Sharp Plug with Schoolbag, R Key and Spindown Dice. Everything working as intended. I use R Key, go to utilize Sharp Plug during the new run and instead of working as intended or it just takes one heart at a time each time and gives me no charges.

Remove Schoolbag, remove and return Sharp Plug, switch active items entirely, nothing fixed it. I didn't have Isaac's heart or anything, and I tried it both partially charged and completely uncharged.

I'm currently pinning this down to a mod issue, potentially Repentagon as it's the only new addition to the roster as of late, but I thought I'd see if anybody else had encountered this, vanilla or otherwise.

I just tested your interaction.

New run, gave myself sharp plug, schoolbag, spindown dice, r key, and a bunch of hp ups.
Found the treasure room, tested the spindown dice + sharp plug, worked as intended.
Used the r key, found the treasure room again, and... still works as intended.

Either you had another item that somehow interfered, or a mod is somehow responsible. At first glance, the game itself is fine.
